CS588 Usability Testing Consent and Release Form
------------------------------------------------

1. The purpose of this study is to identify potential usability
problems in a user interface build as part of a CS588 class project.
 
2. Approximately four - ten subjects will be involved in this study.
 
3. The expected duration of my participation in this research is
thirty minutes.
 
4. The experimental procedure consists of (a) discussing my role in
the experiment and signing this consent form, (b) using an
experimental user interface, (c) filling out a questionnaire, and (d)
providing informal feedback on what I like and dislike about the
interface.
 
5. The foreseeable risks and discomforts of my participation in this
study are the possible fatigue and strain associated with the use of
a computer. I understand that the risks and discomforts will not
exceed those of normal office work involving a computer.
 
6. The expected benefit of this study is to improve the experimental
user interface.
 
7. I will receive no compensation for participating in this
experiment.
 
8. I understand that the experimenter may have to terminate the study
without my consent in the case of a hardware or software failure. I
understand that I am free to deny any answers to specific items or
questions without penalty.
 
9. No report from this study will contain data that can be identified
with me individually. All records from the study will be maintained
confidentially.

10. Possible claims of injury or reaction should be made to Martin
Frank (CS588 Co-Instructor) at (310) 822-1511 ext. 182. I understand
that neither the University of Southern California nor the CS588
instructors nor the experimenters have made any provision for payment
of costs associated with any injury resulting from participation in
this study.

12. You have rights as a research volunteer. Taking part in this
study is completely voluntary. If you do not take part, you will have
no penalty. You may stop taking part in this study at any time with
no penalty.
