Camera ready copy must be submitted as a generic postscript file. In particular, if you use a PC, do not use printer specific drivers, use postscript to file.
Text and figures must be bounded by 4.75 inches wide x 7.5 inches high. Text must be single spaced 11 point roman, preferably 'times roman' font, and should be justified (right and left). First lines of all paragraphs should be indented.
Margins must be 2.25 inches from top and 1.9 inches from left.
Title of paper must begin on the first page 2.25 inches from the top, as for body text. The title should be 15 point bold times roman all caps. Subtitle (if present) begin on the next line, 13 point italic times roman. Leave three blank 13 point lines following the title. Author name(s) should be mixed case, 11 point times roman. Author affiliations should begin on the next line in mixed case, 9 point times roman italic. Email addresses should begin on the next line, also 9 point times roman italic. Notes of funding support should not appear below the title, but rather as a footnote.
Endnotes (rather than footnotes) are generally preferred (except for noting funding source); font should be 9 point times roman. If footnotes are used, they should be 8 point times roman font.
All headings should be flush left (not centered), times roman, as follows:

Figures should be horizontally centered. The caption should be centered under the figure, in 9 point times roman font. Captions should read "Figure n <legend>" where "Figure <n>" is in italic, n is numbered sequentially from 1, and <legend> is in normal (non-italic) font. One blank 11 point line should follow the caption before subsequent text.
Tables should be flush left with a slight indent. The caption should be flush left over the table, in 9 point times roman font. Captions should read "Table n <legend>" where "Table <n>" is in italic, n is numbered sequentially from 1, and <legend> is in normal (non-italic) font. One blank 11 point line should intervene between preceeding text and the table legend.
Both figures and tables must be cited in the text.
References should be in 9 point times roman font, numbered sequentially from 1 in order of author last name, and cited in the body text as "[n]" where n is the referece number. Use whatevercitation style you prefer, but it should be complete. URLs are acceptable as part of the reference, but citation to an authoritative hardcopy version should always be supplied if it exists.
