There is no standard way to implement spoofing so all of these
questions are implementation dependent. I am not aware of any
document which describes spoofing but it is within the charter of this
group to write one (unless the charter changed when I wasn't
looking). However, the chartered document is not really intended to
answer the questions you raise but rather to describe spoofing (and
its pros and cons) at a much higher level...
