vern paxson's thesis (forget the pointer but vern should also be on
this list) has a chapter on measuring bottleneck bandwidth, and it
explains how packet-pair works and also its limitations.

if you want to make experiments on how bandwidth estimators work,
the "dummynet" stuff at http://www.iet.unipi.it/~luigi/ip_dummynet/
is quite appropriate. It has been recently integrated with a packet
filter and it is much easier and flexible than it was 2 years ago.
