In general, yes.

Depends. Some framing overhead is simulated in some wireless
simulation. (It isn't for satellite.) The overhead of
serialisation/channelisation (complex TDMA stuff, say) isn't in
detail.
In general, don't assume any overhead is simulated; assume that it
isn't, and check to see if it is.
